0005676: Dwarves Fail to Respect Burrows When Fleeing Enemy
This only became apparent when I used an alert burrow to prevent Dwarves from walking into a region with Abhorent Muck rainfall. A Nightwing attacked, and everyone scattered in all directions, ignoring the burrow restriction completely. Presumably the walls belowground have prevented this behavior from being observed before now.

This reminds me of the Invisible Fence someone once sold us to keep our dogs in the yard. After we bought it, it turned out that it only really works if you have a real fence to slow them down if they attempt to jump or tunnel. Otherwise they pass over the wire too quickly for the shock collar to make any impact on their behavior.

Burrows don't restrict pathing, they only restrict the areas they will look at when deciding what jobs to perform or what to path to. (And military matters override this.) If you have a burrow that only includes a dwarf's room, dining hall, food stockpile, and workspace, with none of the hallways in between, they will still path through the areas outside their burrow in order to get there. While having some sort of restrict-pathfinding-to-this-area thing would be nice, that is not an intended feature of the current system.

If civilians will leave the Alert burrow at will, it isn't much use. I am not talking about walking over a doorsill between adjoining portions of the burrow. I am talking about running completely out of it and over the horizon, during a lockdown.

I've had numerous dwarves die a painful death at the hands of ambushers and invaders because they run straight past my fort enterance when fleeing, even though my alert restrict all civilians to a burrow inside the fort. I can understand that civilians flee out of burrows when they're threatened by enemies inside it and there's no other way, but they should try to run toward their burrows if they get the chance.
