Could you possibly upload a save to DFFD? It would probably help quite a bit.

I made the mistake of re-saving over it, and it no longer happens. But if I run into it again, I'll be sure to.

PS in case its not obvious, I'm playing with the Ironhand graphics set.

I encountered in many times but I never managed to replicate this (usually it happens with melting ice) and it never survives saving.

If it is related to melting ice could it be caused by 0003868?
Maybe the stone is placed on the unusable bugged tile produced by ice melting, and because of that distance finding algorithm cant reach it and returns error value?
